  Your total benefit cost equals an additional 16.61% of your annual salary.

401(k) Plan:  (Your Company) is pleased to be a partner with you in providing for your retirement.  You may contribute up to 20% of your income before taxes each paycheck, and for every dollar of the first 6% of pay you contribute, the company deposits $.50.  This is an immediate 50% return on your investment!  You are immediately 100% vested in both your contributions and the company match.  (Your company) also makes a discretionary company contribution to your 401(k) account at the end of each year as a percentage of your annual pay.  These contributions begin after you have met certain hours and length of service requirements, and vest at 100% after five years of vesting service.

Paid Time Off Benefits (PTO):  Everyone needs occasional time away from work to re-energize – its an important part of keeping your personal life in balance with your work responsibility.  (Your Company) provides you PTO each year to use for time away from work, during the 2004 calendar year, your PTO (including holidays) is 25 days.

Medical Insurance:  Two excellent medical plan options are available to you and your dependents.  The PPO Medical Plan has no annual deductible for services received from in-network providers, with no need for specialist referrals.  The Comprehensive Medical Plan allows you to use any medical provider, with reimbursement at 80% or 100% after an annual deductible.

Dental Insurance:  You and your dependents have two quality dental options to choose from.  The Preventative Dental Plan provides 100% coverage for checkups, cleanings and routine exams; and 80% coverage for restorative services.  The Comprehensive Dental Plan adds 60% coverage for major services, and 50% orthodontia coverage for dependent children.

Basic & Supplemental Life Benefits:  (Your Company) provides 1x your total compensation in life insurance at no cost to you.  You may elect to purchase coverage of up to 5x your total compensation up to $500,000 at low group premium rates.  You may also purchase up to $20,000 life insurance coverage for your spouse and $10,000 for each child. 

Long Term Disability Benefits:  Long term disability benefits are provided as a source of income protection should you become disabled as a result of a non-work related accident injury or illness.  You may be eligible to receive up to $1,924.00 taxable benefit per month (60% of your monthly salary up to a maximum of $15,000 per month).  Benefits begin after 90 continuous days of disability. 

Flex Plan:  The Health Care, Dependent Care and Transportation Reimbursement Accounts allow you to set aside pre-tax dollars to pay for eligible unreimbursed medical, dental, vision, day care and parking expenses.
